If you are a Physician, Health Care Professional or a Medical Facility all we need is your name address, telephone number and a few small pieces of information to create an account for you. 

We do not require credit checks; the entire procedure can be completed in two minutes by telephone.

If you do not fall under one of the above categories we may require, at our discretion, credit card payment in advance of an order being shipped.
